I’ve never lost mine, but some of my family members have.
Learning from their mistakes, I set aside a place to keep my phone when I’m not using it. I made sure to put it there every time, and soon it became a habit.

When I lose mine I just get a new one.
Yes I have before, and got it back. I just always make sure it is on me. I carry it on my waist with a clip/holder or pocket.
